After you complete online registration, the primary account holder will receive an email with a link to complete CampDoc, typically within 24 hours of completing online registration. If registration occurs well in advance of the program start date, the CampDoc email will be sent at least two weeks prior to the start of the program. All forms must be completed before your child can attend a program.
Required forms include:
- Child information
- Child birth certificate
- Child physical form (must be signed by a physician; previous school age physicals are accepted if signed by a physician)
- Child immunization record (must be signed by a physician)
- Parent and guardian information
- Child emergency information
- Additional authorized pickups
- Medication consent (required if your child needs medication while in our care)
- Allergy management plan (required if your child has diagnosed food allergies)
- Financial documents (tax return, recent paycheck stubs)
- Authorizations
- Parent handbook
Please Note: Medication consent and allergy management plan forms expire one year from the date they are signed by a physician, unless otherwise noted.
Summer Camp:
CampDoc profiles are due by Thursday at 11:59pm before your registered camp week.
Afterschool:
CampDoc profiles are due by Thursday at 11:59pm before the start of school. After school begins, forms must be completed before your child can attend
Summer Camp:
A non-refundable $50 registration fee is due at the time of registration. In addition, a $25 non-refundable weekly deposit is required at registration to hold your child’s spot for each registered week (this deposit is applied toward your balance). Payments are drafted every Thursday before your registered week. Cancellations must be submitted two weeks in advance to avoid being drafted. Payments are non-refundable and not prorated.
Afterschool:
A non-refundable $25 registration fee is due at the time of registration. Payments are drafted every Monday. If payment is not collected within 24 hours, students are suspended until payment is made.
Cancellation requests must be submitted through each child’s participant profile in CampDoc. Registration changes and cancellations must be submitted two weeks in advance. This helps us maintain proper staffing ratios and offer space to families on the waitlist.
Please Note: registration fees and weekly deposits are non-refundable, not prorated, and non-transferable.
Because the Y is a state licensed child care provider, we accept Department of Social Services (DSS) and other third party payment assistance providers.
If your annual household income is less than $60,000, you may qualify for free or reduced childcare. Other families may qualify depending on family size.
